I, the undersigned, acknowledge that I have independently chosen to purchase travel insurance without any recommendation or endorsement from Bon Voyage Boutique Travel (the “Agency”), or have chosen to waive travel insurance coverage. I understand that by declining insurance and choosing to proceed with purchasing independent insurance coverage, I accept the risks associated with this decision. I further understand and agree that the Agency makes no guarantees regarding the coverage, benefits, or suitability of any insurance policy I select and will not provide guidance or assistance in the claims process. I accept full responsibility for declining insurance coverage or managing my insurance policy and any claims directly with the insurance provider. I release and hold harmless Bon Voyage Boutique Travel, its agents, and employees from any liability or expenses arising from cancellation or interruption, emergency medical and transportation, baggage and personal effects, flight and travel accident, rental car physical damage, or any other claims related to my travel insurance decision.
